Each year we update our records by asking parents to fill out a Children’s Ministry Enrollment Form.  We strongly encourage parents to fill out one form for each child in their family that is between Birth and 5th Grade.

You are NOT required to fill out an Enrollment Form to attend Pinedale, but it is extremely beneficial to you and to us when you do.  We use the information off the Enrollment Form to provide teachers with rosters for their class, to make attendance sheets, to be aware of any medical problems or allergies, to make Security Tags, to update Parent Pager, and to send out mailings.

All Enrollment Forms are handled confidentially.
